Phlebotomist Requirements for Registry

Must be 100% Dedicated to the best Possible Patient Care!
Must have at least 1 year Hospital or Outpatient lab experience
Should have a National Certification Not required with verification of adequate experience
Verification of State Certificate for LA, CA, WA, NV and any other states that require State Certification
Be a current Phlebotomist performing at least 10 collections per month
Current CPR & FA Training Card by an approved Organization Ex.. AHA, RedCross, Medic First Aid / HIS Etc.
Current HIPAA Training Certificate within 2 years
Current Blood Borne Pathogen Training Certificate within 2 years
Independent Contractor / application for registry Approval
Signed Code of Conduct
Signed Occupational Exposure Guidelines
Signed W9 on file
Signed BAA
